Atosiban Acetate is an oxytocin receptor antagonist used intravenously to delay imminent preterm labor in pregnant women between 24 and 33 weeks of gestation.
Description
An advanced tocolytic agent that inhibits oxytocin-stimulated uterine contractions by blocking calcium channel activation, thereby inducing uterine quiescence.
Indications
To delay imminent pre-term birth in pregnant adult women with regular uterine contractions, cervical dilation of 1-3 cm, and gestational age from 24 to 33 completed weeks.

Oxytocin Receptor Antagonist
Pharmacology
Binds to myometrial oxytocin receptors, acting as a Gq antagonist to prevent intracellular calcium increase and subsequent uterine contractions.
Mechanism of action
Blocks oxytocin action, reduces cytosolic calcium release, and induces uterine relaxation.
Dosage
Administered intravenously in three stages: initial bolus (6.75 mg), loading infusion (300 mcg/min for 3 hours), followed by subsequent infusion (100 mcg/min up to 45 hours). Total duration not to exceed 48 hours.

Intravenous infusion administered in hospital settings under medical supervision.
Missed dose
Managed by clinical staff.
Side effects
Nausea (very common); headache, dizziness, flushing, vomiting, tachycardia, hypotension, injection site reactions, hyperglycemia (common); fever, insomnia, pruritus, rash (uncommon).
Precautions & warnings
Use with caution in multiple pregnancies, hepatic impairment, or when premature membrane rupture cannot be excluded.
Contraindications
Gestational age <24 or >33 weeks, eclampsia, severe pre-eclampsia, intrauterine fetal death, placenta previa, abruptio placentae, and conditions where pregnancy continuation is hazardous.
Drug interactions
No significant CYP450 interactions. Tested safely with labetalol and betamethasone.
