Bimatoprost 0.03% + Timolol 0.5% Eye Prep is a combination medicine used in the treatment of ocular hypertension and glaucoma. It works by decreasing the production of aqueous humour (fluid in the eye), thereby lowering increased eye pressure.
Description
Bimatoprost 0.03% + Timolol 0.5% Eye Prep is a fixed combination indicated for the reduction of intraocular pressure in adult patients with open-angle glaucoma or ocular hypertension who are insufficiently responsive to monotherapy.
Bimatoprost is a prostaglandin analogue that lowers IOP by increasing aqueous humor outflow through both the trabecular meshwork and uveoscleral routes. Timolol is a non-selective beta-adrenergic receptor antagonist that reduces aqueous humor production. The combination provides additive IOP-lowering effects.
Recommended dose: One drop in the affected eye(s) once daily. Use as advised by your doctor.

Pharmacology
Bimatoprost lowers IOP by increasing aqueous humor outflow through both the trabecular meshwork and uveoscleral routes. It mimics the action of a naturally-occurring prostaglandin, increasing the drainage of aqueous humour out of the eyeball, and may also lower the rate of aqueous formation. Timolol is a non-selective beta-adrenergic receptor antagonist that reduces elevated as well as normal intraocular pressure by decreasing aqueous humor production.
Mechanism of action
Bimatoprost reduces IOP by increasing aqueous humor outflow (trabecular and uveoscleral pathways) and decreasing aqueous production. Timolol blocks beta-adrenergic receptors to reduce aqueous humor production. Together, these mechanisms significantly lower intraocular pressure.
Dosage
Adults: One drop in the affected eye(s) once daily.
Children: Safety and effectiveness in pediatric patients have not been established.
Elderly: No overall differences in safety or effectiveness have been observed between elderly and other adult patients.
Administration
For external use only. Take in the dose and duration as advised by your doctor. Check the label for directions before use. Hold the dropper close to the eye without touching it. Gently squeeze the dropper and place the medicine inside the lower eyelid. Wipe off extra liquid. If you wear soft contact lenses, remove them before using the drops and wait at least 15 minutes before putting them back in. Do not touch the tip of the dropper to any surface or your eye to avoid contamination.
Missed dose
If you miss a dose, apply it as soon as you remember. Skip the missed dose if it is almost time for your next dose. Do not double the dose.
Side effects
Common (in clinical trials):
Ocular hyperemia (approximately 26% of patients), decreased visual acuity, eye discomfort, foreign body sensation, eye pain, pruritus (itching), stinging sensation, burning sensation.
Other:
Darkening of eyelashes (temporary), change in iris color (brown pigmentation with long-term use), eye redness.
Precautions & warnings
- May cause short-term blurring of vision after use; do not drive or use machinery until vision is clear.
- Apply pressure on the corner of the eye (close to the nose) for about 1 minute immediately after instilling the drop.
- Use within 4 weeks of opening the bottle.
- May cause eyelashes to grow longer, darker, and thicker (not harmful).
- May cause brown pigmentation of the iris with long-term use (permanent but not harmful).
- Caution in patients with cardiovascular disease or pulmonary disorders (due to systemic absorption of timolol).
Contraindications
Contraindicated in patients with known hypersensitivity to any ingredient of this formulation.
Drug interactions
- Potential for bradycardia when administered concomitantly with oral calcium channel blockers, guanethidine, beta-adrenergic blocking agents, parasympathomimetics, anti-arrhythmics, and digitalis glycosides.
- Concomitant ocular medications should be administered at least 5 minutes apart from this Eye Drops.
Food interactions

Use in pregnancy
May be unsafe during pregnancy. Limited human studies available; animal studies have shown harmful effects on the developing baby. Should not be used during pregnancy unless clearly necessary. Consult your doctor.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Probably safe during breastfeeding (if prescribed). Limited human data suggests the drug does not pose significant risk to the baby. However, animal studies show Bimatoprost is excreted in rat milk and Timolol is excreted in human milk. Caution advised.
Pediatric use
Safety and effectiveness in pediatric patients have not been established.
Geriatric use
No overall differences in safety or effectiveness have been observed between elderly and other adult patients.
Renal / hepatic impairment
No interaction found/established.
Overdose effects
Overuse may cause eye irritation, redness, or systemic beta-blocker effects (bradycardia, hypotension). Consult your doctor if symptoms occur.
Storage conditions
Store below 25°C in a dry place protected from light. Keep out of reach of children. Solution can be used up to 28 days after first opening.
