Description

Dasatinib is a tyrosine kinase inhibitor that inhibits multiple tyrosine kinases including BCR-ABL, SRC family, c-KIT, PDGFR-β, and EPHA2. It is effective in imatinib-resistant CML patients. It stops cancer cell growth and proliferation and induces apoptosis.

Pharmacology

Dasatinib is a tyrosine kinase inhibitor that inhibits multiple kinases including BCR-ABL, SRC family, c-KIT, PDGFR-β, and EPHA2. It is effective in imatinib-resistant CML patients.

Mechanism of action

Dasatinib inhibits BCR-ABL tyrosine kinase, preventing cancer cell proliferation and inducing apoptosis.

Dosage

Adult - CML (Chronic Phase): 100 mg once daily orally.

Adult - CML (Accelerated or Blast Phase): 140 mg once daily orally.

Adult - ALL: 140 mg once daily orally.

May be taken with or without food.

Administration

Take at the same time each day with or without food. Swallow the tablet whole, do not chew, crush or break it.

Missed dose

If you miss a dose of Dasatinib, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose. Do not double the dose.

Side effects

Common: Diarrhea, headache, fatigue, nausea, rash, muscle pain, pleural effusion, edema, neutropenia, thrombocytopenia, anemia.

Serious: Myelosuppression, bleeding, fluid retention (pleural effusion, pericardial effusion), QT prolongation, pulmonary hypertension.

Precautions & warnings

Myelosuppression: Monitor blood counts regularly; Fluid retention: Monitor weight and dyspnea; Bleeding risk; QT prolongation: ECG monitoring; Avoid pregnancy; Monitor liver function tests.

Contraindications

Hypersensitivity; Pregnancy; Lactation.

Drug interactions

Strong CYP3A4 inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir): May increase dasatinib levels; dose reduction may be needed.

Strong CYP3A4 inducers (rifampin, phenytoin, carbamazepine): May decrease dasatinib levels; dose increase may be needed.

Antacids and H2 receptor antagonists: May decrease absorption; take 2 hours before or after dasatinib.

Food interactions

It is not known whether it is safe to consume alcohol with Dasatinib. Please consult your doctor.

Use in pregnancy

Pregnancy Category D. Dasatinib is unsafe during pregnancy as there is definite evidence of risk to the developing baby. May be used in life-threatening situations if benefits outweigh risks.

Pregnancy Category: D — always consult a doctor before taking during pregnancy.

Use in lactation

Dasatinib is unsafe during breastfeeding. Data suggests that the drug may cause toxicity to the baby.

Pediatric use

Safety and effectiveness in pediatric populations have not been established.

Geriatric use

No specific dosage adjustment is necessary for elderly patients. However, caution should be exercised.

Renal / hepatic impairment

Renal impairment: Probably safe in kidney disease. Dose adjustment may not be needed. Please consult your doctor.

Hepatic impairment: Safe in liver disease. No dose adjustment recommended.

Overdose effects

Overdose may lead to myelosuppression, nausea, diarrhea, and other side effects. Seek medical attention.
