Description
Sodium phosphate can be used in the management of hypophosphataemia. It also acts as a mild osmotic laxative when given orally as dilute solutions or by the rectal route. Phosphate enemas or concentrated oral solutions are used for bowel cleansing before surgery or endoscopy procedures.

Pharmacology
Sodium phosphate can be used in the management of hypophosphataemia. It also acts as a mild osmotic laxative when given orally as dilute solutions or by the rectal route.
Mechanism of action
Sodium phosphate draws water into the bowel, creating an osmotic effect that stimulates peristalsis and helps cleanse the colon.
Dosage
Adult (Colon cleansing - Early morning procedure): First dose at 7 AM, second dose at 7 PM (each dose: 15 mL solution + 250 mL clear liquid, repeat 3 times within 20 minutes).
Adult (Constipation - ≥12 years): 20-45 mL.
Child (10-11 years): 10-20 mL.
Child (5-9 years): 5-10 mL.
Administration
Drink at least 3 glasses (approximately 250 mL each) of clear liquids between doses. Drink plenty of fluids to prevent dehydration.
Missed dose
If you miss a dose, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose.
Side effects
Common: Bloating (31-41%), nausea (26-30%), abdominal pain (23-25%), vomiting (4-9%).
Other: Dizziness, headache, seizures, renal impairment, increased BUN/creatinine, acute renal failure, acute phosphate nephropathy, anaphylaxis, rash, pruritus, urticaria, throat tightness, bronchospasm, dyspnea.
Precautions & warnings
Caution in severe renal insufficiency (CrCl <30 mL/min), CHF, ascites, unstable angina, gastric retention, ileus, acute bowel obstruction, pseudo-obstruction, severe chronic constipation, bowel perforation, acute colitis, toxic megacolon, gastric bypass surgery, hypomotility syndrome.
Contraindications
CHF, renal failure or clinically significant impaired renal function, congenital or acquired megacolon, bowel/GI obstructions, ascites, perforation of ileus, active inflammatory bowel disease, allergy/hypersensitivity to sodium phosphate.
Drug interactions
Calcium channel blockers, diuretics, ACE inhibitors, ARBs, NSAIDs, lithium: May affect electrolyte levels.
Polyethylene glycol (PEG) preparations: Concurrent use may be dangerous; not recommended.
Food interactions
Take clear liquids (water, tea/coffee without milk, carbonated/non-carbonated soft drinks, fruit juices) - not colored red or purple. Avoid alcohol.
Use in pregnancy
Use during pregnancy is not recommended (potential harm to fetus from phosphate absorbed across the placenta).
Pregnancy Category: N/A — always consult a doctor before taking during pregnancy.
Use in lactation
Use during lactation is not recommended (potential harm to infant from phosphate excreted in breast milk).
Pediatric use
Not to be taken by children under 5 years of age.
Geriatric use
Use with extreme caution in the elderly, frail, or debilitated patients.
Renal / hepatic impairment
Caution in renal impairment; dose adjustment may be needed.
Overdose effects
Overdose may cause electrolyte imbalance, dehydration, cardiac arrhythmias. Seek medical attention.
Storage conditions
Store in a cool and dry place, protected from light. Keep out of reach of children.
