Domperidone + Esomeprazole is a combination medicine used for the treatment of gastroesophageal reflux disease (GERD), acid reflux, indigestion, nausea and vomiting. Domperidone helps the stomach empty faster and Esomeprazole reduces stomach acid production.
Description
Domperidone is a prokinetic that increases stomach and intestinal movement to help food pass more easily through the digestive tract. It also blocks dopamine receptors to reduce nausea and vomiting. Esomeprazole is a proton pump inhibitor (PPI) that suppresses the final step of gastric acid production by blocking the H+/K+-ATPase enzyme in gastric parietal cells. Together, these two medicines relieve heartburn, bloating and stomach discomfort.
Indications
Gastroesophageal reflux disease (GERD), acid reflux, heartburn, indigestion (dyspepsia), flatulence, feeling of fullness after eating, nausea and vomiting.

Pharmacology
Domperidone is a dopamine antagonist with anti-emetic and gastrokinetic properties. It increases lower esophageal pressure, improves antroduodenal motility and accelerates gastric emptying. Esomeprazole suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase enzyme in the gastric parietal cell.
Mechanism of action
Domperidone increases stomach and intestinal movement to help food pass more easily and reduces nausea by blocking dopamine receptors. Esomeprazole stops gastric acid production by blocking the H+/K+-ATPase enzyme. Together, these two medicines prevent acid reflux and relieve heartburn, bloating and stomach discomfort.
Dosage
Adults: Usually twice daily (morning and evening) or as prescribed by the physician. Available as capsules or tablets containing Domperidone 10-30mg and Esomeprazole 20-40mg. Should be taken 30 minutes before meals on an empty stomach.
Administration
Take 30 minutes before meals on an empty stomach. Swallow the capsule or tablet whole; do not chew or crush. Follow the prescribed dose and schedule.
Missed dose
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not double the dose.
Side effects
Common side effects: Headache, diarrhea, nausea, flatulence, abdominal pain, constipation, dry mouth. Rare side effects: Dizziness, drowsiness, allergic reactions.
Precautions & warnings
Caution during pregnancy. Not recommended during breastfeeding. Caution in elderly patients and those with cardiac problems or electrolyte abnormalities. Caution while driving as it may cause dizziness. Long-term use may increase fracture risk. Avoid alcohol.
Contraindications
Hypersensitivity to domperidone or esomeprazole or any other proton pump inhibitor. Moderate to severe liver impairment, existing QT interval prolongation, significant cardiac conduction abnormalities, congestive heart failure. Gastrointestinal bleeding, mechanical obstruction, perforation. Prolactinoma.
Drug interactions
Avoid concomitant use with clopidogrel as esomeprazole may reduce platelet inhibition. Avoid with atazanavir and nelfinavir. Ketoconazole, erythromycin, clarithromycin may inhibit CYP3A4 metabolism of domperidone and prolong QTc interval. Other interactions: Warfarin, diazepam, tacrolimus, digoxin, methotrexate, voriconazole, rifampin.
Food interactions
Caution advised when consuming alcohol as it may increase stomach acid production and worsen symptoms. Avoid excessive grapefruit juice.
Use in pregnancy
Use with caution during pregnancy. Animal studies have shown harmful effects on the fetus, but human studies are limited.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Not recommended during breastfeeding. Limited data suggests it does not pass into breast milk in significant amounts, but caution is required.
Pediatric use
Safety and efficacy not established in children. Avoid use in children under 12 years.
Geriatric use
Elderly patients (over 60 years) have higher risk of cardiac effects (QT prolongation). Use the lowest effective dose of domperidone.
Renal / hepatic impairment
Caution in renal impairment. Dose reduction of domperidone may be needed in severe renal impairment. Caution in liver impairment; contraindicated in moderate to severe liver disease.
Overdose effects
Symptoms of overdose: Drowsiness, irregular heartbeat, stomach discomfort. Seek medical attention immediately if overdose is suspected.
Storage conditions
Store below 30°C in a cool and dry place, protected from light. Keep out of reach of children.
