Fluticasone Propionate Nasal prep

Introduction: Fluticasone Propionate Nasal spray is a steroid that relieves the symptoms of allergic rhinitis such as runny nose, sneezing and sinus discomfort. It works by blocking the production of certain chemical messengers that cause inflammation and allergies.

How it works: Fluticasone Propionate utilises a fluorocarbothioate ester linkage at the C17 position. It has potent vasoconstrictive and anti-inflammatory activity, but weak HPA inhibitory effect when applied topically.

Important Information: This spray should be used regularly for full benefit. Normally, blow your nose gently before using it, keep one nostril closed while applying the medicine into the other and then sniff hard to make sure it reaches deep into the nose.

• Seasonal and perennial allergic rhinitis
• Prevention of recurrence of nasal polyps following surgical removal
• Chronic rhinosinusitis with or without nasal polyps in adults (93 mcg/spray strength)

Nasal Steroid Preparations

Synthetic corticosteroid

Fluticasone Propionate is a synthetic trifluorinated corticosteroid with anti-inflammatory activity. It is an aqueous suspension of microfine Fluticasone Propionate for topical administration to the nasal mucosa by means of a metering, atomizing spray pump. Each bottle provides 120 metered sprays.

Fluticasone Propionate binds to glucocorticoid receptors and reduces the activity of inflammatory cells and mediators, resulting in decreased nasal inflammation and allergy symptoms.

Adult Dose (50 mcg/spray):

• 2 sprays in each nostril once daily, preferably in the morning.
• May be increased to 2 sprays in each nostril twice daily if needed.
• Maximum 200 mcg (4 sprays)/day.

Children (4-11 years):

• 1 spray in each nostril once daily.
• May be increased to 1 spray in each nostril twice daily if needed.
• Maximum 100 mcg (2 sprays)/day.

Nasal Polyps (Adult):

• 4 sprays (200 mcg) into each nostril 1-2 times daily for at least 4-6 weeks.

Under 4 years: Safety and efficacy not established.

Fluticasone Propionate (93 mcg/spray):

• 1 or 2 sprays per nostril twice daily.
• Maximum: 2 sprays in each nostril twice daily (total 744 mcg).

Instructions for using nasal spray:

• Shake the bottle gently and remove the dust cover.
• Before first use or if not used for a week or more, press several times until a fine mist appears.
• Gently blow nose to clear nostrils.
• Close one nostril and insert the applicator into the other nostril.
• Tilt head forward slightly and keep spray upright. Press firmly to release a spray while breathing in.
• Breathe out through mouth.

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and follow your regular schedule.

Common Side Effects:

• Nasal irritation
• Nosebleeds
• Headache
• Drying of nose and throat
• Unpleasant taste and smell

Precautions:

• Avoid deep breathing while using the spray (medication may go to throat).
• Do not share the bottle with anyone else.
• Avoid in patients with recent nasal ulcers, nasal surgery, or nasal trauma.
• Potential risk increases with larger doses - avoid exceeding recommended doses.

Contraindications:

• Hypersensitivity to any of its ingredients

Drug Interactions:

• CYP3A4 inhibitors (e.g., ritonavir) may increase plasma concentrations of fluticasone.

Food Interactions: No interaction found/established.

Use in Pregnancy: May be unsafe during pregnancy. Animal studies have shown harmful effects. Your doctor will weigh the benefits and risks.

Use in Lactation: Probably safe during breastfeeding. Limited data suggests the drug does not represent any significant risk to the baby.

Pediatric Use: Dosage available for children 4-11 years. Safety and efficacy not established for children under 4 years.

Geriatric Use: No specific dosage adjustment is necessary for elderly patients.

Renal Impairment: No interaction found/established (minimal systemic absorption due to nasal use).

Hepatic Impairment: No interaction found/established (minimal systemic absorption due to nasal use).

Overdose: Overdose is unlikely when used as directed. Excessive use may cause nasal irritation.

Storage: Store below 30°C, away from light and moisture. Keep out of reach of children.
