Gramicidin + Neomycin + Polymixin B (Eye prep)

Introduction: Gramicidin + Neomycin + Polymixin B Eye drops is used for bacterial eye infections such as conjunctivitis, keratitis, blepharitis, corneal ulcer and keratoconjunctivitis.

How it works: Neomycin is an aminoglycoside antibiotic that binds to 30S ribosomal subunits, inhibiting protein synthesis. Polymyxin B disrupts the bacterial cytoplasmic membrane of mostly gram-negative organisms. Gramicidin is bactericidal against gram-positive organisms.

Important Information: Instill 1-2 drops into the affected eye every 4 hours for 7 to 10 days. In severe infections, dosage may be increased to 2 drops every hour.

Antibiotic Combination

Neomycin is bactericidal for many gram-positive and gram-negative organisms. It is an aminoglycoside antibiotic which inhibits protein synthesis by binding with ribosomal RNA and causing misreading of the bacterial genetic code. Polymyxin B is bactericidal for a variety of gram-negative organisms. It increases the permeability of the bacterial cell membrane by interacting with the phospholipid components of the membrane. Gramicidin is bactericidal for a variety of gram-positive organisms. It increases the permeability of the bacterial cell membrane to inorganic cations by forming a network of channels through the normal lipid bilayer of the membrane.

Neomycin inhibits protein synthesis, Polymyxin B disrupts cell membrane and Gramicidin increases cell membrane permeability to kill bacteria.

Adult & Child Dose:

• 1-2 drops into the affected eye every 4 hours
• For 7 to 10 days
• In severe infections, may be increased to 2 drops every hour

For ophthalmic use only. Hold the dropper close to the eye without touching it. Gently squeeze the dropper and place the medicine inside the lower eyelid.

If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and follow your regular schedule.

Common Side Effects:

• Allergic reactions - itching, swelling, conjunctival erythema
• Local irritation
• Anaphylaxis (rare)

Precautions:

• Prolonged use may result in overgrowth of non-susceptible organisms including fungi
• Bacterial resistance may develop
• If purulent discharge, inflammation or pain becomes aggravated, discontinue and consult doctor
• Allergic cross-reactions may occur (kanamycin, paromomycin, streptomycin, gentamicin)

Contraindications:

• Hypersensitivity to any of its components

Drug Interactions: No significant interactions known.

Food Interactions: No interaction found/established.

Use in Pregnancy: Pregnancy Category C. Use only if clearly needed.

Use in Lactation: Unknown if excreted in human milk. Use with caution.

Pediatric Use: Safety and efficacy in pediatric patients have not been established.

Geriatric Use: No specific dosage adjustment is necessary for elderly patients.

Renal & Hepatic Impairment: Minimal systemic absorption due to topical use - no known restrictions.

Overdose: Overdose is unlikely when used as directed. Excessive use may cause eye irritation.

Storage: Store in a cool and dry place.
