Description
Hydrocortisone Acetate 0.5% + Neomycin Sulphate 0.5% Topical is a combination used for burns, eczema, infected wounds & ulcers, pyogenic & seborrhoeic dermatitis and other corticosteroid-responsive dermatoses. Hydrocortisone is a corticosteroid with anti-inflammatory and immunosuppressive effects. Neomycin is an aminoglycoside antibiotic that kills bacteria by inhibiting protein synthesis.

Pharmacology
Hydrocortisone is a corticosteroid that provides anti-inflammatory action by suppressing migration of polymorphonuclear leukocytes and reversing increased capillary permeability. Neomycin is an aminoglycoside antibiotic that binds to 30S ribosomal subunits of bacterial ribosome, inhibiting protein synthesis and generating errors in genetic code transcription causing cell death. It acts against many gram-negative aerobes and some gram-positive aerobes.
Mechanism of action
Hydrocortisone reduces inflammation and itching; Neomycin kills bacteria by inhibiting protein synthesis.
Dosage
Adult Dose (Topical):
- Apply cream/ointment onto affected area 2-3 times as needed
Child Dose: Safety and efficacy not established
Administration
For external use only; clean and dry affected area before application; use in the dose and duration as advised by your doctor.
Missed dose
If you miss a dose, apply it as soon as possible; if it is almost time for your next dose, skip the missed dose.

Precautions & warnings
Precautions:
- Caution in CHF, hypertension, DM, epilepsy, elderly, patients on prolonged therapy

- Viral/fungal infections
- Tubercular or syphilitic lesions
- Bacterial infections (unless used with appropriate chemotherapy)
- Hypersensitivity
Drug interactions
Drug Interactions:
- Hydrocortisone: Thiazides may enhance hyperglycaemia and hypokalaemia; NSAIDs increase peptic ulcer or GI bleeding; anticoagulant response altered; antidiabetic and antihypertensive doses need to be increased; ethanol may enhance gastric mucosal irritation; carbamazepine, phenytoin, primidone, barbiturates and rifampicin reduce efficacy
- Neomycin: Additive nephrotoxic and neurotoxic effect with other aminoglycosides, bacitracin, cisplatin, vancomycin, amphotericin B, polymyxin B, colistin; enhanced toxicity with potent diuretics; may impair absorption of other drugs; may reduce efficacy of oral contraceptives; may enhance effect of non-depolarising muscle relaxants
