Description

Neomycin binds to 30S ribosomal subunit to inhibit bacterial protein synthesis. Prednisolone decreases inflammation by inhibition of polymorphonuclear leukocyte migration and reversal of increased capillary permeability. Polymyxin B causes leakage of bacterial membrane to produce bactericidal effect.

Pharmacology

Neomycin binds to 30S ribosomal subunit to inhibit bacterial protein synthesis. Prednisolone inhibits polymorphonuclear leukocyte migration and reverses capillary permeability to reduce inflammation and suppresses immune system. Polymyxin B causes bacterial membrane leakage to produce bactericidal effect.

Mechanism of action

Treats infection by inhibiting bacterial protein synthesis and reduces inflammation.

Dosage

Adult Dose (Eye Drops):

  • 1-2 drops every 3-4 hours; acute infections may require every 30 minutes; reduce frequency as infection is controlled
  • For lid treatment: 1-2 drops every 3-4 hours, close eye and rub excess on lids

Pediatric Dose: 1-2 drops 2-4 times daily

Administration
  • Do not touch dropper tip to eye
  • Instill in lower eyelid
  • Wipe off extra liquid
Missed dose

If you miss a dose, use it as soon as you remember; skip if it's almost time for next dose; do not double the dose.

Side effects

Side Effects:

  • Elevation of intraocular pressure (glaucoma and optic nerve damage)
  • Posterior subcapsular cataract formation
  • Delayed wound healing
  • Fungal and viral infections of cornea (with long-term steroid use)
Precautions & warnings
  • Prolonged corticosteroid use may result in optic nerve damage and vision defects
  • Monitor intraocular pressure regularly with prolonged use
  • Risk of fungal and viral infections
Contraindications
  • Epithelial herpes simplex keratitis
  • Vaccinia, varicella
  • Viral diseases of cornea and conjunctiva
  • Mycobacterial infection of eye
  • Fungal diseases of ocular structures
  • Hypersensitivity
  • After uncomplicated removal of corneal foreign body
Drug interactions
  • Synergistic effects with penicillins
  • Additive toxicity with neurotoxic, ototoxic or nephrotoxic drugs
  • May decrease absorption of digoxin and methotrexate
  • May increase effects of oral anticoagulants
