Description
Tedizolid Phosphate is a prodrug that is converted by phosphatases to Tedizolid, the microbiologically active moiety. It binds to the bacterial 50S ribosomal subunit, inhibiting protein synthesis. It is effective against Gram-positive bacteria including methicillin-resistant and susceptible Staphylococcus aureus (MRSA/MSSA).

Pharmacology
Tedizolid Phosphate is converted by phosphatases to Tedizolid, which binds to the bacterial 50S ribosomal subunit, inhibiting protein synthesis. It is effective against methicillin-resistant Staphylococcus aureus (MRSA), Streptococcus pyogenes, S. agalactiae, S. anginosus Group, and Enterococcus faecalis.
Mechanism of action
Inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit.
Dosage
Adult Dose (Oral/IV):
- 200 mg once daily for 6 days
- May be taken with or without food
- IV: Administer as IV infusion over 1 hour; do not give as IV push or bolus
Administration
- Oral: Take with or without food
- IV: Administer as IV infusion over 1 hour
- Do NOT give as IV push or bolus
- Not for intra-arterial, IM, IT, intraperitoneal, or SC administration
Missed dose
If you miss a dose, take it as soon as possible up to 8 hours before the next scheduled dose; if less than 8 hours remain, wait until the next dose.
Side effects
Common Side Effects (2-10%):
- Nausea (8%), Headache (6%)
- Diarrhea (4%), Hemoglobin <10.1 g/dL (3.1%)
- Vomiting (3%), Platelets <112×10³/mm³ (2.3%)
- Dizziness (2%)
Rare (<2%):
- Anemia, palpitations, tachycardia
- Blurred vision, infusion-related reactions
- Insomnia, pruritus, urticaria, dermatitis
Precautions & warnings
- Consider alternative therapy in patients with neutropenia (neutrophils <1,000/mm³)
- Discontinue if C. difficile-associated diarrhea is suspected
- Not evaluated in diabetic foot infections, ischemic ulcers, bone/joint infections, necrotizing infections
- May affect driving ability
Contraindications
- Hypersensitivity to tedizolid or any component
Drug interactions
- Rosuvastatin: Increased AUC and Cmax
- BCRP substrates (methotrexate, topotecan): Increased plasma concentrations
Food interactions
Caution is advised when consuming alcohol with Tedizolid Phosphate; consult your doctor.
Use in pregnancy
Use in Pregnancy: No adequate controlled studies; use only if clearly needed; consult your doctor.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation

In overdose, discontinue tedizolid and provide general supportive treatment.
