Description
Acetazolamide is a carbonic anhydrase inhibitor that reduces intraocular pressure in glaucoma and serves as an adjuvant treatment for edema and seizures.
Indications
Indicated in open angle glaucoma, secondary glaucoma, and as an adjuvant in the treatment of edema and epilepsy.

Pharmacology
Acetazolamide inhibits carbonic anhydrase enzyme, decreasing the secretion of aqueous humor in the eye and dropping intraocular pressure. In the CNS, it retards abnormal paroxysmal neuronal discharges.
Mechanism of action
Inhibition of carbonic anhydrase results in reduced hydrogen ion secretion at renal tubules and decreased aqueous humor formation in ciliary bodies of the eye.
Dosage
Adults: Initially 250 mg (1 tablet) 4 times daily. Acute intraocular pressure: 2 tablets (500 mg) initially followed by 1 tablet every 6 hours. Children: 0.5 to 2 tablets in divided doses.
Administration
Administer orally with water, preferably with meals to lessen GI distress.
Missed dose
Take as soon as remembered unless it is almost time for the next scheduled dose. Do not take double doses.
Side effects
Common: Headache, drowsiness, dizziness, fatigue, anorexia, polyuria, insomnia, gastrointestinal upset, and paresthesia.
Precautions & warnings
Monitor serum electrolytes for hypokalemia or hyponatremia and observe for metabolic acidosis during long-term therapy.
Contraindications
Contraindicated in marked kidney/liver dysfunction, decreased blood sodium/potassium levels, hyperchloremic acidosis, and chronic noncongestive angle closure glaucoma.
Drug interactions
Increased risk of hypokalemia with other diuretics. May increase plasma levels of phenytoin, carbamazepine, and digitalis glycosides.
Food interactions
Taking with food helps reduce gastrointestinal irritation.
Use in pregnancy
Pregnancy Category C. Use only if clearly needed and benefits outweigh potential risks to fetus.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Excreted in small amounts in human milk; caution should be exercised when administered to a nursing woman.
Pediatric use
Dose must be carefully titrated based on body weight/age under specialist supervision.
Geriatric use
Elderly patients are more susceptible to electrolyte imbalance and metabolic acidosis.
Renal / hepatic impairment
Contraindicated in severe renal impairment (CrCl < 10 mL/min) and severe hepatic insufficiency.
Overdose effects
Overdose may cause electrolyte imbalance, metabolic acidosis, and CNS effects. Treatment is supportive.
