Description
Calcium Dobesilate is a vasoactive drug that presumes effects on endothelial integrity, capillary permeability, and blood viscosity. It decreases capillary hyperpermeability, reduces platelet aggregation, and lowers serum viscosity, which improves blood circulation and blood supply to tissues and organs.
Calcium Dobesilate is indicated for hemorrhoidal syndrome, microcirculation disorders of arteriovenous origin, clinical signs of chronic venous insufficiency in the lower limbs (pain, cramps, paresthesia, edema, stasis, dermatosis), and particular microangiopathy such as diabetic retinopathy. It is also indicated as adjuvant therapy in superficial thrombophlebitis.

Pharmacology
Calcium Dobesilate acts selectively on the capillary walls, regulating their physiological functions of resistance and permeability. It lowers blood viscosity and restores normal blood flow. This action is beneficial in all cases of capillary fragility due to metabolic disturbances (constitutional or acquired), surgical stress, or induced by certain medicines.
Mechanism of action
Calcium Dobesilate acts on capillary walls to reduce permeability and fragility. It lowers blood viscosity to improve blood flow and reduces platelet aggregation.
Dosage
Adults: 1-2 capsules (500 mg) daily with main meals.
Treatment duration: Generally between a few weeks and several months, depending on the disease and its evolution.
Dosage adjustment: Individualized based on the severity of the case.
Severe renal insufficiency (dialysis): Dose should be reduced.
Administration
Take with meals. Swallow capsules whole; do not chew or crush. Follow the dose and duration as advised by your physician.
Missed dose
If you miss a dose, skip it and continue with your normal schedule. Do not double the dose.
Side effects
Rare side effects:
• Gastrointestinal disorders (nausea, diarrhea)
• Skin reactions
• Fever
• Articular pain (joint pain)
• Very rare: Agranulocytosis (0.32/million patients)
These reactions are generally spontaneously reversible after treatment withdrawal. In case of gastrointestinal disorders, reduce the dose or temporarily withdraw treatment. In case of skin reactions, fever, articular pain, or changes in blood formula, stop treatment and inform your physician.
Precautions & warnings
- Risk of agranulocytosis: In very rare cases, calcium dobesilate may induce agranulocytosis. If symptoms such as high fever, oral cavity infections (tonsillitis), sore throat, anogenital inflammation, or accompanying symptoms occur, the patient must immediately inform their physician. Blood formula and leucogram should be checked without delay and treatment discontinued.
- Severe renal insufficiency (dialysis): Dosage should be reduced.
- Patients experiencing flu-like symptoms (cough, sore throat, fever) should seek medical care as soon as possible.

Use in pregnancy
Adequate information on use during pregnancy is not available (no human or animal studies). It is not known whether calcium dobesilate crosses the placental barrier. Use only if anticipated therapeutic benefit outweighs potential risk to the fetus.
Use in lactation
Calcium dobesilate enters maternal milk in very low quantities (0.4 µg/ml after intake of 3×500 mg). As a precaution, either treatment or breastfeeding should be stopped.
Pediatric use
Safety and efficacy in pediatric patients have not been established.
