Description
Clobetasol Propionate + Salicylic Acid topical has a combination of medicines that are used to treat eczema and psoriasis. It helps to provide relief from redness, itching and swelling. It removes dead skin cells and helps in softening the skin. Clobetasol Propionate + Salicylic Acid topical should be used in the dose and duration as advised by your doctor. Wash your hands before using this medicine. It is advised to check the label for directions before use. It is for external use only. Do not use this medicine more than the recommended dose.
Clobetasol Propionate + Salicylic Acid topical is a combination of two medicines: Clobetasol and Salicylic Acid, which treats eczema and psoriasis. Clobetasol is a steroid which blocks the production of certain chemical messengers (prostaglandins) that make the skin red, swollen and itchy. Salicylic Acid is a keratolytic medicine which breaks down keratin clumps, removes dead skin cells and softens the skin. It also enhances the absorption of Clobetasol into the skin.

- Eczema (nummular eczema, hand eczema, eczematous dermatitis)

Pharmacological class
Topical Corticosteroid + Keratolytic
Pharmacology
Clobetasol Propionate is a high potency corticosteroid. Corticosteroids decrease inflammation by stabilizing leukocyte lysosomal membranes, preventing release of destructive acid hydrolases from leukocytes; inhibiting macrophage accumulation in inflamed areas; reducing leukocyte adhesion to capillary endothelium; reducing capillary wall permeability and edema formation; decreasing complement components; antagonizing histamine activity and release of kinin from substrates; and reducing fibroblast proliferation, collagen deposition, and subsequent scar tissue formation. Salicylic acid has a potent keratolytic action which softens and destroys the stratum corneum by increasing endogenous hydration which causes the horny layer of the skin to swell, soften, and then desquamate.
Mechanism of action
Clobetasol Propionate reduces inflammation, redness, and itching by blocking inflammatory mediators. Salicylic Acid breaks down keratin clumps, removes dead skin cells, and softens the skin.
Dosage
Adult Dose (Topical): Apply a thin layer to the affected skin areas twice daily and rub in gently & completely. Therapy should be discontinued when control has been achieved. If no improvement is seen within 2 weeks, reassessment of diagnosis may be necessary. Should not be used with occlusive dressing. Treatment beyond 2 consecutive weeks and exceeding 50 gm/week is not recommended.
Pediatric Dose (under 12 years): Not recommended.
Administration
This medicine is for external use only. Use it in the dose and duration as advised by your doctor. Check the label for directions before use. Clean and dry the affected area and gently massage the ointment.
Missed dose
If you miss a dose of Clobetasol Propionate + Salicylic Acid topical, apply it as soon as you remember. However, if it is almost time for your next application, skip the missed dose. Do not double the dose.
Side effects
Common Side Effects:
- Skin peeling
- Thinning of skin
- Application site reactions (burning, stinging, itching and redness)
- Dryness
Serious Side Effects (long-term/large area use): Hypercortisolism (adrenal suppression), local atrophy, systemic absorption, salicylate toxicity (tinnitus, dizziness, deafness).
Precautions & warnings
- Use with caution on lesions close to the eye.
- Prolonged use over extensive areas may lead to adrenal suppression; pediatric patients are more susceptible.
- Appropriate antimicrobial therapy should be used when treating inflammatory lesions that have become infected.
- Should not be used with occlusive dressing.
- Avoid contact with broken or inflamed skin.
- Caution during pregnancy and lactation.
Contraindications
- Hypersensitivity to Clobetasol Propionate
- Primary infected bacterial or fungal skin lesions (if no anti-infective agent is used simultaneously)
- Primary cutaneous viral infections (herpes simplex, vaccinia, varicella)
- Tuberculous skin lesions
- Dermatoses in children under one year of age (including dermatitis and diaper eruptions)
- Salicylate hypersensitivity or sensitivity to any other ingredient
Drug interactions
There has been no report of interaction with Clobetasol Propionate ointment and cream. There are no known interactions of Salicylic Acid when used as indicated. However, topical salicylic acid may increase the absorption of other topically applied medicines. Concomitant use of Salicylic Acid Ointment and other topical medicines on the same area of skin should therefore be avoided.
Food interactions
No food interactions have been found or established.
Use in pregnancy
[CONSULT YOUR DOCTOR] Clobetasol Propionate + Salicylic Acid topical may be unsafe to use during pregnancy. Topical administration of corticosteroids to pregnant animals can cause abnormalities of fetal development. Pregnancy Category C.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
[SAFE IF PRESCRIBED] Clobetasol Propionate + Salicylic Acid topical is probably safe to use during breastfeeding. Limited human data suggests that the drug does not represent any significant risk to the baby.
Pediatric use
Not recommended for use in pediatric patients under 12 years of age. Contraindicated in dermatoses in children under one year of age.
Geriatric use
Limited information is available regarding dose adjustment for elderly patients.
Renal / hepatic impairment
For topical use, no dose adjustment is required in renal or hepatic impairment. However, systemic absorption may occur when used over large areas.
Overdose effects
Acute overdosage is very unlikely to occur. In the case of chronic overdosage or misuse, features of hypercortisolism may appear. Symptoms of systemic salicylate poisoning (tinnitus, dizziness, deafness) have been reported after application to large areas of skin and for prolonged periods.
Storage conditions
Store in a cool and dry place, protected from light. Keep out of reach of children.
