Doripenem is a broad-spectrum carbapenem with potent antibacterial activity against aerobic and anaerobic gram-positive and gram-negative bacteria. It inhibits bacterial cell wall synthesis.

Pharmacology

Doripenem is a broad-spectrum carbapenem active against aerobic and anaerobic gram-positive and gram-negative bacteria. It binds to penicillin-binding proteins and inhibits bacterial cell wall synthesis.

Mechanism of action

Doripenem binds to penicillin-binding proteins, inhibiting the final transpeptidase step of peptidoglycan synthesis in bacterial cell walls, leading to bacterial death.

Dosage

Adult Dose:

  • Complicated intra-abdominal infections: 500 mg IV every 8 hours for 5-14 days
  • Complicated UTI and pyelonephritis: 500 mg IV every 8 hours for 10 days

Renal Dose: CrCl 30-50: 250 mg every 8 hours; CrCl 11-29: 250 mg every 12 hours.

Side effects

  • Nausea
  • Injection site reactions (pain, swelling, redness)
Precautions & warnings
  • Usually given in hospital for serious infections.
  • Inform doctor if allergic to penicillin or taking seizure medication.
  • Inform doctor if diarrhea, severe abdominal pain or blood in stools.
  • Liver and kidney function may be monitored.

Drug interactions

Probenecid increases plasma concentration. May decrease valproic acid levels, increasing seizure risk.

Consuming alcohol with Doripenem does not cause any harmful side effects.
