Lasmiditan is used for the acute treatment of migraine with or without aura in adults. It is a potent and selective 5-HT1F receptor agonist that inhibits trigeminal nerve pathway activation.
Description
Lasmiditan is a potent and selective agonist of the 5-HT1F receptor. 5-HT1F receptors are found in both the central and peripheral nervous system and contribute to hyperpolarization of nerve terminals and inhibition of trigeminal neuronal activity. Lasmiditan's agonism at these receptors inhibits the firing of trigeminal nerves responsible for migraine headache pain. It has virtually no affinity for other 5-HT receptor subtypes or monoamine receptors.

Pharmacology
Lasmiditan inhibits trigeminal nerve firing through agonism at 5-HT1F receptors. It works without relying on cerebral vasoconstriction (supporting the neurogenic hypothesis as an alternative to the vascular hypothesis).
Mechanism of action
Inhibits trigeminal neuronal activity by activating 5-HT1F receptors, relieving migraine pain.
Dosage
Adult Dose (Oral):
- 50 mg, 100 mg, or 200 mg as needed
- No more than one dose in 24 hours
- Do not drive or operate machinery for at least 8 hours after dosing
- May be taken with or without food
Hepatic Impairment: Severe (Child-Pugh C): Not recommended

- Swallow tablet whole; do not chew, crush or break
- Take as needed
- Do not drive for at least 8 hours after dosing

- Hypersensitivity to Lasmiditan or its components

- Serotonergic drugs (SSRIs, SNRIs, TCAs, MAO inhibitors): Risk of serotonin syndrome
- Heart rate-lowering drugs: Additional heart rate reduction
- P-gp and BCRP substrates: Avoid use
Food interactions
Alcohol may cause sedation and CNS depression when used with Lasmiditan.
Use in pregnancy
Use in Pregnancy: No adequate data in pregnant women. Animal studies have shown adverse effects. Use only if clearly needed.
Use in lactation
Use in Lactation: No data on presence in human milk. Excreted in rat milk at ~3 times maternal plasma levels. Use with caution.
Pediatric use
Safety and effectiveness not established in pediatric patients.
Geriatric use
Insufficient data in patients 65 years and older.
Renal / hepatic impairment
Hepatic Impairment: Mild-moderate: No dose adjustment; Severe (Child-Pugh C): Not recommended.
Overdose effects
