Description

The mechanism of action of Lurasidone in the treatment of schizophrenia and bipolar depression is unknown. However, its efficacy could be mediated through a combination of central dopamine Type 2 (D2) and serotonin Type 2 (5HT2A) receptor antagonism. Administration with food substantially increases absorption.

Pharmacology

Lurasidone acts through D2 and 5HT2A receptor antagonism, helping control symptoms of schizophrenia and bipolar depression.

Mechanism of action

Modulates dopamine and serotonin receptors in the brain to affect thoughts and mood.

Dosage

Schizophrenia: 40 mg once daily initially; may increase to 80 mg/day; max 160 mg/day

Bipolar Depression: 20 mg once daily initially; max 120 mg/day

Hepatic Impairment: Moderate (Child-Pugh B): 20 mg/day; max 80 mg/day; Severe (Child-Pugh C): 20 mg/day; max 40 mg/day

Renal Impairment: CrCl <50 mL/min: 20 mg/day; max 80 mg/day

Administration
  • Swallow tablet whole; do not chew, crush or break
  • Take with food (at least 350 calories)
  • Take at the same time each day
Missed dose

If you miss a dose, take it as soon as you remember. If it's almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

  • Dementia-related psychosis
Drug interactions
  • Reduce dose to half with moderate CYP3A4 inhibitors
  • May need to increase dose with moderate CYP3A4 inducers
  • Avoid grapefruit and grapefruit juice
Food interactions

Alcohol consumption with Lurasidone is unsafe.

Use in pregnancy

Use in Pregnancy: Lurasidone is generally considered safe during pregnancy (Category B); animal studies have shown low or no adverse effects to the developing baby.

Pregnancy Category: B — always consult a doctor before taking during pregnancy.

Use in lactation

Use in Lactation: Lurasidone is probably unsafe during breastfeeding; may pass into breast milk. Should not be used by nursing mothers.

Pediatric use

Safety and effectiveness not established in pediatric patients.

Geriatric use

Avoid use in elderly patients with dementia-related psychosis; consider lower starting dose and slower titration in patients with cardiovascular or cerebrovascular disease.

Renal / hepatic impairment

Renal Impairment: Caution in kidney disease; dose adjustment needed when CrCl <50 mL/min.

Hepatic Impairment: Caution in liver disease; dose adjustment needed in moderate-severe hepatic impairment.
