Mesna Disulfide is used to prevent ifosfamide-induced hemorrhagic cystitis. It reduces the risk of bladder inflammation and bleeding in patients receiving ifosfamide.
Description
Mesna Disulfide reacts with urotoxic metabolites (acrolein) of ifosfamide to detoxify them. It prevents urothelial toxicity by reacting with thiol groups in the renal tubule. In the kidney, it is reduced to the free thiol compound Mesna, which binds to urotoxic metabolites and inactivates them.

- Prophylaxis of ifosfamide-induced hemorrhagic cystitis
- Prevention of urothelial toxicity associated with ifosfamide or cyclophosphamide

Pharmacology
Mesna Disulfide detoxifies ifosfamide's urotoxic metabolites (acrolein) by reacting with thiol groups in the renal tubule, reducing the risk of bladder inflammation and bleeding.

Dosage
Adult Dose (IV):
- 240 mg/sq.meter IVP with ifosfamide dose, 15 minutes before and 4 and 8 hours after ifosfamide administration
- Adjust dose if ifosfamide dose changes
Administration
- Administered IV by doctor or nurse only; do not self-administer
- Can be given as IV bolus, intermittent infusion, or continuous infusion

Not generally applicable (hospital administered); consult doctor if needed.

Precautions & warnings
- Drink at least 1 litre of fluid daily
- Seek immediate medical attention for severe allergic reactions (rash, itching, breathing difficulty, swelling of mouth/face/lips/tongue)
- Caution when driving or operating machinery (may cause drowsiness, dizziness, blurred vision)
- Will be monitored for blood or protein in urine during treatment
Contraindications
- Hypersensitivity to thiol-containing compounds
Drug interactions
- Incompatible in vitro with cisplatin and nitrogen mustard
