Description
Mesna Disulfide is used to prevent ifosfamide-induced hemorrhagic cystitis. But, its not used to reduce the risk of blood in the urine due to other medical conditions. It only helps to reduce the risk of inflammation and bleeding in the bladder, who receive ifosfamide. Mesna Disulfide is given under the supervision of doctor. It is taken in a dose as advised by the doctor. The duration of treatment varies on the basis of your need and response to treatment. You should take it exactly as your doctor has advised. Taking it in the wrong way or taking too much can cause very serious side effects. It may take several weeks or months for you to see or feel the benefits but do not stop taking it unless your doctor tells you to. Some common side effects of this medicine include nausea, headache, and lightheadedness. It is advised to drink at least 1 liter of fluid daily when you are on treatment with this medicine. You should seek immediate medical attention if you experience severe allergic reactions like rash, itching, breathing difficulty, chest tightness and swelling of the mouth, face, lips, or tongue while taking this medicine. Regular blood tests are required to check your blood cells along with kidney, liver, and blood uric acid levels. Before taking it, tell your doctor if you have autoimmune disorder, liver, or kidney problems or are taking any medicines to treat infections. Many other medicines can affect, or be affected by, this medicine so let your healthcare team know all medications you are using. This medicine is not recommended during pregnancy or while breastfeeding. The use of effective contraception by both males and females during treatment is important to avoid pregnancy. Mesna Disulfide helps to reduce the risk of inflammation and bleeding in the bladder, who receive ifosfamide (a medicine used to treat cancer).

Mechanism of action
Mesna is used to prevent urothelial toxicity associated with oxazaphosphorine, ifosfamide or cyclophosphamide. It acts in the kidney; reacting with thiol groups of urotoxic metabolites (e.g. acrolein) of ifosfamide and cyclophosphamide. It is used as a mucolytic in the management of some respiratory tract conditions e.g. cystic fibrosis where other mucolytics have failed. It acts by reducing the viscosity of pulmonary secretions; the drug's free sulfhydryl group is thought to reduce disulfide linkages of mucoproteins.
Dosage
Adult Dose: Intravenous Prophylaxis against urothelial toxicity With Ifosfamide 240 mg/sq.meter (if receiving 1.2 g/sq.meter ifosfamide dose) IVP 15 minutes before & 4 & 8 hours after ifosfamide admin OR 240 mg/sq.meter (if receiving 1.2 g/sq.meter ifosfamide dose) IVP 15 minutes before & 480 mg/sq.meter PO 2 & 6 hours after ifosfamide admin Adjust dose accordingly if ifosfamide dose changes Also given by alternative IVP regimens, intermittent infusion or continuous infusion-
Administration

Side effects
Common: • Nausea • Headache • Lightheadedness • Lethargy • Abdominal pain • Flushing (sense of warmth in the face, ears, neck and trunk) • Diarrhea • Rash • Fever • Flu-like symptoms
Precautions & warnings
[UNSAFE] Mesna Disulfide may cause side effects which could affect your ability to drive. As patients undergoing treatment with Mesna Disulfide may experience undesirable effects such as lightheadedness, lethargy, dizziness, and blurred vision which could affect the ability to drive. Drink at least 1 litre of fluid daily while you are receiving mesna. Seek immediate medical attention if you experience severe allergic reactions like rash, itching, breathing difficulty, chest tightness, and swelling of the mouth, face, lips, or tongue while taking mesna. Do not drive or operate machinery as mesna may cause drowsiness, dizziness, blurred vision, or light headedness. Tell your doctor if you are pregnant or planning to become pregnant or are breastfeeding. You will be monitored for blood or protein in urine during the treatment of mesna. Do not take if allergic to mesna or any of its ingredients (thiol containing compound)
Contraindications
Hypersensitivity to thiol-containing compounds.
Drug interactions
Mesna is incompatible in vitro with cisplatin and nitrogen mustard.

Use in pregnancy

Use in lactation
[UNSAFE] Mesna Disulfide is unsafe to use during breastfeeding. Data suggests that the drug may cause toxicity to the baby.
Renal / hepatic impairment
