Monosulfiram is an ectoparasiticide used in the treatment and prevention of scabies. It is active against the burrowing mite Sarcoptes scabiei. Applied topically as a 25% solution.

Pharmacology
Monosulfiram is an ectoparasiticide active against the scabies mite (Sarcoptes scabiei). It acts on the mite's nervous system to inactivate them.
Mechanism of action
Treats scabies by inactivating the scabies mite.
Dosage
Adult Dose (Topical):
- 25% solution: Dilute with 2-3 parts water before application
- Apply diluted solution to entire body (except face and scalp)
- Allow 10 minutes to dry, then dress
- In difficult cases, repeat for 2-3 days
Prophylaxis: Use daily for as long as infection risk remains
Administration
- For external use only
- Dilute with 2-3 parts water before use
- Bathe with hot water and soap, dry skin thoroughly
- Pay special attention to skin folds
- Allow to dry naturally or blot with towel
Missed dose
Not generally applicable (scheduled use); consult doctor if needed.
Side effects
Side Effects:
- Erythematous rash (rare)
- Idiosyncratic reactions
Precautions & warnings
- Do not apply near naked flame (solution is flammable)
- Due to chemical relationship with disulfiram, avoid alcohol before and for at least 48 hours after application
- Avoid contact with eyes and mucous membranes
Contraindications
- Patients who have previously shown an idiosyncratic response to its application

Caution with alcohol: Avoid alcohol before and for at least 48 hours after Monosulfiram application (may cause disulfiram-like reaction).

Overdose effects
Overdose (ingestion): Flushing, dyspnea, headache, dizziness, nausea, vomiting, drowsiness; tachycardia and hypotension may occur; symptomatic treatment (oxygen, cardiac stimulants, IV sodium thiosulfate)
Storage conditions
Store below 35°C (room temperature); do not store in cold place (crystals may deposit); flammable, keep away from naked flame.
