Description
Tolterodine is a competitive, specific muscarinic receptor antagonist which exhibits selectivity for the urinary bladder over salivary glands. It reduces bladder contractions, thereby improving OAB symptoms.

Pharmacology
Tolterodine is a competitive, specific muscarinic receptor antagonist with selectivity for the urinary bladder over salivary glands. A major active metabolite (5-hydroxymethyl derivative) exhibits a similar pharmacological profile. In extensive metabolizers, this metabolite contributes significantly to the therapeutic effect.
Mechanism of action
Blocks muscarinic receptors, reducing bladder contractions.
Dosage
Adult Dose:
- Extended-release capsule: 4 mg once daily; may lower to 2 mg once daily
- Film-coated tablet: 2 mg twice daily
- Significantly reduced hepatic or renal function: 1 mg twice daily or 2 mg once daily
- CYP3A4 inhibitors: 2 mg once daily
Administration
- May be taken with or without food
- Swallow whole
- Take at the same time each day
Missed dose
If you miss a dose, take it as soon as possible; if almost time for next dose, skip it; do not double the dose.

- Caution in bladder outflow obstruction
- Caution in gastrointestinal obstructive disorders

- Uncontrolled narrow-angle glaucoma
- Hypersensitivity
Drug interactions
- Ketoconazole, itraconazole, miconazole, erythromycin, clarithromycin, cyclosporine, vinblastine: increased tolterodine plasma concentrations
Food interactions
Alcohol consumption should be cautioned.
Use in pregnancy
Use in Pregnancy: No adequate studies; use only if potential benefit justifies risk; consult your doctor.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Not known if excreted in human milk; decide whether to discontinue nursing or drug; consult your doctor.

Geriatric use
No overall differences in safety observed between older and younger patients
Renal / hepatic impairment
Renal Impairment: Severe (GFR<30 ml/min): 1 mg twice daily
Hepatic Impairment: 1 mg twice daily
Overdose effects
Symptoms: Severe central anticholinergic effects.
Treatment: ECG monitoring; supportive care.

Store in a cool and dry place; protect from light; keep out of reach of children.
