Indications
- Hypertension
- Chronic stable angina
- Coronary Artery Disease (CAD)
- Primary hypercholesterolemia (heterozygous familial and non-familial)
- Mixed dyslipidemia (Fredrickson Types IIa and IIb)
- Hypertriglyceridemia (Fredrickson Type IV)
- Primary dysbetalipoproteinemia (Fredrickson Type III)
- Homozygous Familial Hypercholesterolemia (HoFH)
Dosage
Amlodipine:
- Hypertension: 5 mg once daily (maximum 10 mg)
- Elderly/Hepatic impairment: Start with 2.5 mg once daily
- Chronic stable/vasospastic angina: 5-10 mg daily
Atorvastatin:
- Hypercholesterolemia & mixed dyslipidemia: Start with 10 mg daily (range 10-80 mg)
- Homozygous FH: 10-80 mg daily
- Renal impairment: No dose adjustment needed
- Hepatic dysfunction: Exposure is increased in moderate-severe disease

Precautions & warnings
Serious Precautions:
- Patients with severe obstructive coronary artery disease may develop increased frequency of angina or MI when starting calcium channel blocker therapy or increasing dose
- Report any muscle pain, tenderness, or weakness to your doctor
- Report signs of liver damage (abdominal pain, dark urine, yellow skin/eyes)
- Do not use during pregnancy and breastfeeding
- Contraindicated in active liver disease or unexplained liver enzyme elevations (>3x ULN)
- Use with caution in patients with heart failure
- Caution in severe aortic stenosis
