Bambuterol Hydrochloride

Bambuterol Hydrochloride is a long-acting beta2-adrenergic receptor agonist used in the treatment of asthma and chronic obstructive pulmonary disease (COPD). It is a prodrug of terbutaline that is converted to the active metabolite terbutaline in the body.

Bambuterol stimulates beta2 receptors, producing relaxation of bronchial smooth muscle, inhibition of release of endogenous spasmogens, inhibition of edema caused by endogenous mediators, and increased mucociliary clearance. It relaxes the muscles in the airways, widening them and making breathing easier.

Common side effects include headache, palpitations, tremor, muscle cramp, insomnia, fatigue, and nausea.

• Other lung diseases where bronchospasm is a complicating factor

Bambuterol is converted to terbutaline in the blood and acts on beta2 receptors to produce bronchodilation.

Bambuterol is converted to terbutaline and selectively stimulates beta2-adrenergic receptors, which relaxes bronchial smooth muscle and widens airways. It inhibits the release of endogenous spasmogens and improves mucociliary clearance.

Adults:

• Initial dose: 10 mg daily (at bedtime)
• Dose increase: May be increased to 20 mg after 1-2 weeks depending on clinical effect

Children (2-5 years): 10 mg (10 ml syrup) daily

Children (6-12 years): 10 mg (10 ml syrup) daily; may be increased to 20 mg if needed

Hepatic Impairment (Severe): Not recommended

Renal Impairment (CrCl <50 mL/min): Initial dose should be halved

Take this medicine in the dose and duration as advised by your doctor. Swallow the tablet whole; do not chew, crush, or break. May be taken with or without food, but take at a fixed time (usually at bedtime).

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not double the dose.

• In severe renal impairment (CrCl <50 mL/min), halve the initial dose
• Caution in patients with history of diabetes, thyroid disease, high blood pressure, heart disease, liver or kidney problems
• Caution in patients with family history of glaucoma
• Monitor serum potassium levels in patients with severe asthma
• Caution during the first trimester of pregnancy
• Caution when driving or operating machinery (may cause dizziness or tremor)

• Severe hepatic impairment
• Liver cirrhosis
• Severely impaired liver function

• Corticosteroids, diuretics, xanthine derivatives: Increase risk of hypokalemia
• Succinylcholine: May prolong action
• MAO inhibitors: Caution required
• Beta-blockers: May cause adverse effects
• Sympathomimetics: Additive effects

[CONSULT YOUR DOCTOR] Bambuterol Hydrochloride may be unsafe during pregnancy (Category C). Animal studies have shown harmful effects on the developing baby. Caution is recommended during the first trimester.

[CONSULT YOUR DOCTOR] It is not known whether bambuterol or its metabolites are excreted in human milk. Terbutaline is excreted in breast milk in small amounts. Caution is recommended in nursing mothers.

Safety and efficacy in children under 2 years have not been established.

No dose adjustment needed for elderly patients, but renal function should be monitored.

[CAUTION] Caution in liver disease. Not recommended in severe hepatic impairment.

No case of bambuterol overdose has been reported in humans. Overdose may result in high levels of terbutaline causing headache, anxiety, tremor, nausea, muscle cramps, palpitations, tachycardia, and cardiac arrhythmias. Hyperglycemia and lactacidosis may occur. High doses of beta2-agonists may cause hypokalemia.

Store below 30°C, away from light and moisture. Keep out of reach of children.
