Description

Faropenem is a penem class of antimicrobial and has bactericidal activity against a variety of gram-negative, gram-positive and anaerobic bacteria. Faropenem is bactericidal, with a strong affinity for the high molecular penicillin binding proteins (PBPs) of the cell wall, which is essential for the multiplication of bacilli; it, thus, acts by inhibiting the cell wall synthesis.

Faropenem shows broad antibacterial activity against both aerobic and anaerobic Gram-positive and Gram-negative bacteria. Faropenem is highly stable against various beta lactamases and binds preferentially to the PBPs, 2 and 1A, of Escherichia coli.

Pharmacology

Faropenem is a penem class of antimicrobial and has bactericidal activity against a variety of gram-negative, gram-positive and anaerobic bacteria. Faropenem is bactericidal, with a strong affinity for the high molecular penicillin binding proteins (PBPs) of the cell wall, which is essential for the multiplication of bacilli; it, thus, acts by inhibiting the cell wall synthesis.

Mechanism of action

Faropenem exerts bactericidal activity by inhibiting bacterial cell wall synthesis. It has broad antibacterial activity against gram-positive, gram-negative and anaerobic bacteria.

Dosage

Adult Dose (Oral):

  • Upper Respiratory Tract Infections: 150 mg tid (may increase to 200 mg tid)
  • ENT Infections: 200 mg tid (may increase to 300 mg tid)
  • Lower Respiratory Tract Infections: 200 mg tid (may increase to 300 mg tid)
  • Skin Infections: 150 mg tid (may increase to 200 mg tid)
  • Genito-urinary Infections: 200 mg tid (may increase to 300 mg tid)
  • Gynecological Infections: 150 mg tid (may increase to 200 mg tid)

Elderly: Start with 150 mg and monitor for adverse effects.

Administration

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Faropenem may be taken with or without food.

Missed dose

If you miss a dose of Faropenem, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

Precautions & warnings

Precautions:

  • Caution in patients with past history of hypersensitivity to penicillin, cephem or carbapenem drugs.
  • Caution in patients with family history of atopy.
  • Caution in patients with renal impairment; dosage should be reduced or interval between doses increased.
  • Caution in geriatric patients; start with 150 mg and monitor.
  • Caution in patients with poor oral intake or poor general state (vitamin K deficiency symptoms may occur).
  • If diarrhea and loose bowel movements appear, stop the medicine, monitor, and take appropriate measures.

Contraindications:

  • Hypersensitivity
  • Anaphylactic reactions to beta-lactams
Drug interactions

Drug Interactions:

  • Imipenem + Cilastatin: Increases blood concentration of faropenem (animal studies).
  • Furosemide: Increases kidney toxicity of faropenem (animal studies).
  • Sodium Valproate: Joint use with carbapenem drugs may reduce valproic acid concentration and cause recurrence of epileptic fits.
Food interactions

Food Interactions: It is not known whether it is safe to consume alcohol with Faropenem; consult your doctor.

Use in pregnancy

Use in Pregnancy: Safety during pregnancy not established; should be given only if benefit outweighs risks.

Pregnancy Category: C — always consult a doctor before taking during pregnancy.

Use in lactation

Use in Lactation: Faropenem is excreted in human milk; should be given only if benefits outweigh risks.

Pediatric use

Geriatric use

Geriatric Use: Half-life is prolonged (due to decline in kidney functions); start with 150 mg and monitor.

Renal / hepatic impairment

Renal Impairment: Plasma concentrations increased and half-life extended; dosage should be reduced or interval between doses increased.

Hepatic Impairment: No specific dosage adjustment necessary.

Overdose effects

Overdose: No specific information on treatment of overdosage; in the event of overdose, discontinue and provide general supportive treatment until renal elimination takes place.
