Description

Imeglimin Hydrochloride is an oral antidiabetic medication used for the treatment of Type 2 Diabetes Mellitus. It amplifies glucose-stimulated insulin secretion (GSIS) and preserves β-cell mass. It also enhances insulin action by inhibiting hepatic glucose output and improving insulin signaling in both liver and skeletal muscle. At a cellular and molecular level, Imeglimin corrects mitochondrial dysfunction, a common underlying element of T2D pathogenesis.

Pharmacology

Imeglimin's mechanism involves dual effects: (a) amplification of glucose-stimulated insulin secretion (GSIS) and preservation of β-cell mass; and (b) enhanced insulin action, including inhibition of hepatic glucose output and improvement in insulin signalling in liver and skeletal muscle. At a cellular level, it corrects mitochondrial dysfunction, rebalances respiratory chain activity (partial inhibition of Complex I and correction of Complex III activity), reduces reactive oxygen species formation, and prevents mitochondrial permeability transition pore opening.

Mechanism of action

Controls blood sugar by amplifying glucose-stimulated insulin secretion, preserving β-cell mass, inhibiting hepatic glucose output, and correcting mitochondrial dysfunction.

Dosage

Adult Dose (Oral):

  • Usual dosage: 1000 mg (2 tablets) twice daily (morning and evening)
  • Take with food

Renal Dose: Safe in kidney disease; no dose adjustment recommended

Child Dose: Not recommended

Administration

Take with food; swallow whole tablet; do not chew, crush or break; follow the dose and duration as advised by your doctor.

Missed dose

If you miss a dose, take it as soon as possible; if it is almost time for your next dose, skip the missed dose and go back to regular schedule; do not double the dose.

Precautions & warnings

Precautions:

  • It is not known whether Imeglimin alters the ability to drive
  • Hypoglycemic symptoms may occur; be cautious when working at heights, driving, or operating dangerous machinery
  • If hypoglycemia occurs, take sugar-containing foods or sugar (if taking alpha-glucosidase inhibitor, take glucose)
  • Dose reduction may be required for patients with renal impairment (eGFR <15 mL/min/1.73 m2)

  • Hypersensitivity
  • Severe ketosis, diabetic coma or pre-coma
  • Type 1 diabetes (for emergency hyperglycemia)
  • Renal failure, severe renal or hepatic impairment
  • Cardiac failure, CHF, IDDM
  • Pregnancy, lactation
Drug interactions

Drug Interactions:

  • Thiazide diuretics, corticosteroids, phenothiazines, OC, sympathomimetics, niacin, Ca channel blockers, isoniazid may exacerbate loss of glycemic control
  • ACE inhibitors may reduce fasting blood glucose
  • May increase serum level with cimetidine
  • Iodinated contrast agents may increase risk of metformin-induced lactic acidosis
Food interactions

Food Interactions: It is unsafe to consume alcohol with Imeglimin Hydrochloride.

Use in pregnancy

Use in Pregnancy: Not enough reliable information; should be used in pregnant women only if expected therapeutic benefits outweigh possible risks.

Use in lactation

Use in Lactation: Not enough reliable information; transfer to milk reported in animal studies; breastfeeding must be discontinued.
