Description
Nepafenac is a non-steroidal anti-inflammatory prodrug converted to amfenac which putatively inhibits prostaglandin biosynthesis. It is used for post-operative eye pain and inflammation following cataract surgery.
Indications
- Post-operative eye pain and inflammation following cataract surgery
- Prevention of post-operative macular edema in diabetic patients

Pharmacology
Nepafenac is an NSAID prodrug converted to amfenac which inhibits prostaglandin biosynthesis to reduce eye pain and inflammation.
Mechanism of action
Relieves eye pain and inflammation by inhibiting prostaglandin synthesis.
Dosage
Adult Dose (Eye Drops):
- 0.1%: 1 drop 3 times daily, 1 day prior to cataract surgery, on day of surgery and through first 2 weeks
- 0.3%: 1 drop once daily, 1 day before surgery, on day of surgery and through first 2 weeks; additional drop 30-120 minutes before surgery
- Prevention of macular edema (0.1%): 1 drop 3 times daily, 1 day before surgery, on day of surgery and through first 6 weeks
Administration
- Do not touch dropper tip to eye
- Instill in lower eyelid
- Wipe off extra liquid
- Apply pressure on corner of eye (near nose) for 1 minute after instillation
- Wait 5-10 minutes before next medication
- Remove contact lenses (reinsert after 15 minutes)
Missed dose
If you miss a dose, use it as soon as you remember; skip if it's almost time for next dose; do not double the dose.
Side effects
Common Side Effects:
- Application site irritation
- Eye irritation
- Blurred vision
- Watery or dry eyes
Precautions & warnings
- Consult doctor if no improvement in 2 weeks
- Caution while driving or using machines (may cause blurred vision)
- Inform doctor if on blood thinners or have bleeding problems
- Use within 4 weeks of opening
Contraindications
- Hypersensitivity
- Hypersensitivity to other NSAIDs
Drug interactions
- Topical NSAIDs & corticosteroids
- May increase risk of hemorrhage with drugs that prolong bleeding time
Food interactions
No food interactions (eye drops).
Use in pregnancy
Use in Pregnancy: Nepafenac may be unsafe during pregnancy; animal studies show evidence of fetal harm.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Information regarding use during breastfeeding is not available; consult doctor.
Pediatric use
Safety and efficacy not established in children under 10 years.
Geriatric use
Elderly may require dose adjustment.
Renal / hepatic impairment
Renal Impairment: Caution in kidney disease; dose adjustment may be needed.

Overdose effects
