Description
This medicine is a combination of antacids (Calcium Carbonate and Sodium Bicarbonate) and an alginate (Sodium Alginate). It neutralizes excess stomach acid and forms a protective raft of alginic acid gel that floats on the stomach contents, effectively preventing gastro-esophageal reflux for up to 4 hours. The raft protects the esophagus from acid, pepsin and bile.
Indications
Gastric reflux, heartburn, indigestion, flatulence associated with gastric reflux, heartburn of pregnancy, and all cases of epigastric and retrosternal distress where the underlying cause is gastric reflux.

Pharmacology
The mode of action is physical and does not depend on absorption into systemic circulation. Upon ingestion, it reacts rapidly with gastric acid to form a raft of alginic acid gel that floats on stomach contents, impeding gastro-esophageal reflux for up to 4 hours. Calcium Carbonate and Sodium Bicarbonate rapidly neutralize excess stomach acid.
Mechanism of action
Sodium Alginate reacts with gastric acid to form a protective gel raft that prevents acid reflux into the esophagus. Calcium Carbonate and Sodium Bicarbonate neutralize excess stomach acid to provide fast relief from heartburn and indigestion. The combined action provides rapid and prolonged relief.
Dosage
Oral Suspension: Adults and children over 12 years: 10-20 mL after meals and at bedtime, up to four times a day. Children 6 to 12 years: 5-10 mL after meals and at bedtime, up to four times a day. Chewable Tablets: Adults and children 12 years and over: 2-4 tablets after meals and at bedtime, up to four times a day.
Administration
Take with food (after meals and at bedtime). Shake suspension well before use. Chewable tablets should be chewed thoroughly.
Missed dose
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not double the dose.
Side effects
Common side effects: Constipation, flatulence, stomach cramps, belching. Rare side effects: Allergic reactions (urticaria, bronchospasm), anaphylactic reactions. Overdose may cause alkalosis, hypercalcemia, acid rebound, milk-alkali syndrome or constipation.
Precautions & warnings
Each 20 mL dose contains 254.5 mg sodium (11.06 mmol). Caution needed in patients on restricted salt diet (e.g., congestive cardiac failure, renal impairment). Each 20 mL contains 260 mg calcium. Caution needed in patients with hypercalcemia, nephrocalcinosis and recurrent calcium-containing renal calculi. Consult doctor if symptoms persist >7 days.
Contraindications
Hypersensitivity to active ingredients or any excipients.
Drug interactions
Maintain 2-hour interval with other medicines, especially: Tetracyclines, Digoxin, Fluoroquinolones, Iron salts, Ketoconazole, Neuroleptics, Thyroid hormones, Penicillamine, Beta-blockers, Glucocorticoids, Chloroquine, Bisphosphonates and Estramustine.
Food interactions
Certain foods may reduce calcium absorption. Drink adequate water.
Use in pregnancy
Can be used during pregnancy if clinically needed. Clinical studies in 500+ pregnant women showed no malformative or feto/neonatal toxicity.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Can be used during breastfeeding. No effects have been shown in breastfed newborns/infants of treated mothers.
Pediatric use
Not recommended for children under 6 years. Children 6-12 years should use under medical advice.
Geriatric use
No dose modifications necessary for elderly patients.
Renal / hepatic impairment
Caution in patients with severe renal impairment and those on restricted salt diet. Caution in patients with hypercalcemia, nephrocalcinosis and recurrent calcium-containing renal calculi.
Overdose effects
In the event of overdose, symptomatic treatment should be given. The patient may notice abdominal distension. Overdose may cause alkalosis or hypercalcemia.
Storage conditions
Store below 30°C in a cool and dry place, protected from light. Keep out of reach of children. Do not refrigerate or freeze.
