Description

Tulobuterol is a direct-acting sympathomimetic with selective action on β2-receptors. It is available orally, as an inhaler and as a transdermal patch. It is given as the hydrochloride by mouth and the base as transdermal formulation and inhalation.

Pharmacology

Tulobuterol is a direct-acting sympathomimetic with selective action on β2-receptors. The primary pharmacological action of beta-adrenergic drugs is to stimulate adenyl cyclase, the enzyme which catalyzes the formation of cyclic AMP from ATP. The cyclic AMP thus formed mediates the cellular response that results in bronchodilation.

Mechanism of action

Stimulates β2-receptors, relaxing bronchial smooth muscle and widening airways.

Dosage

Adult Dose (Oral):

  • Bronchodilation: Initially 1-2 mg twice daily; increase to 2 mg three times daily if necessary
  • Tablets: Start 1 mg twice daily; may increase to 2 mg twice daily after 7-10 days; may increase to 6 mg/day if necessary

Child Dose (Oral):

  • 1-6 years: 0.25-0.5 mg twice daily
  • 6-10 years: 0.5-1 mg twice daily
  • 10-14 years: 1-1.5 mg twice daily
  • Syrup: 40-80 mcg/kg/day in two divided doses
Administration
  • Make sure skin is clean and dry
  • Remove protective liner of the patch; do not touch sticky side
  • With sticky side down, place patch evenly onto skin
  • Do not wear more than one patch at a time
  • Use one new patch each day, changing the place to avoid skin irritation
Missed dose

If you miss a dose, skip it and continue with your normal schedule; do not double the dose.

  • Administer cautiously to cardiac patients, especially those with arrhythmias, coronary insufficiency, or myocardial ischemia

  • Hypersensitivity to sympathomimetic amines or any formulation components
Drug interactions
  • Digoxin: increased risk of arrhythmia
  • Xanthines, corticosteroids, and diuretics: hypokalaemia
