Fluorometholone 0.1% + Tetrahydrozoline Hydrochloride 0.025% Eye prep

Fluorometholone 0.1% + Tetrahydrozoline Hydrochloride 0.025% Eye prep is used for keratitis, allergic and inflammatory conditions of the eye, iridocyclitis, iritis, acute and chronic conjunctivitis, scleritis, episcleritis, and post-op conditions after strabotomy, cataract and glaucoma operations.

Fluorometholone suppresses the migration of polymorphonuclear leukocytes and reversal of increased capillary permeability thus decreasing inflammation. Tetrahydrozoline may stimulate alpha-adrenergic receptors in the arterioles of the conjunctiva; produces vasoconstriction, thus relieving redness of the eye.

• Keratitis, Iridocyclitis, Iritis: Used for inflammatory conditions of the eye.
• Acute and Chronic Conjunctivitis: Used for allergic non-infectious conjunctivitis and keratitis.
• Post-operative Conditions: Used for irritative conditions after strabotomy, cataract and glaucoma surgery.

Ophthalmic Steroid preparations

Corticosteroid + Alpha-agonist (Decongestant)

Fluorometholone is thought to act by the induction of phospholipase A2 inhibitory proteins which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor arachidonic acid. On the other hand, Tetrahydrozoline is an alpha agonist which constricts the conjunctival blood vessels and thereby relieves the redness of the eye.

Fluorometholone reduces inflammation; Tetrahydrozoline constricts conjunctival blood vessels to relieve redness of the eye.

Adult Dose (Ophthalmic):

• Instill 1 drop into the conjunctival sac 2-3 times daily.
• Dose may be increased to 1 drop hourly for first one to two days in severe cases.

Child Dose: Safety and effectiveness in children below 2 years have not been established.

For external use only. Shake well before use. Hold the dropper close to the eye without touching it. Place the medicine inside the lower eyelid. Wipe off extra liquid.

If you miss a dose of Fluorometholone 0.1% + Tetrahydrozoline Hydrochloride 0.025% Eye prep, apply it as soon as possible; however, if it is almost time for your next dose, skip the missed dose.

Side Effects:

• Mild burning sensation
• Reversible increase in intraocular pressure
• Reactive hyperemia/rebound effect
• Cataract and corneal defects
• Glaucoma
• Systemic effects (chronic use)

Precautions:

• Prolonged use in pregnant women and infants should be avoided.
• Use with caution in patients under treatment with MAO inhibitors.
• Do not apply while wearing soft contact lenses; remove lenses before application and do not reinsert earlier than 15 minutes after use.
• Avoid prolonged use.

Contraindications:

• Viral infection of cornea and conjunctiva caused by Herpes simplex etc
• Fungal infection of the cornea and conjunctiva
• Tuberculous infections
• Ulcerous processes of the cornea
• Hypersensitivity to any ingredients

Drug Interactions:

• MAO Inhibitors (Isocarboxazid, phenelzine, toloxatone, tranylcypromine): May interact with tetrahydrozoline; caution required.

Food Interactions: No interaction found/established.

Use in Pregnancy: No adequate and well-controlled studies; should be used only if potential benefits justify potential risk to the fetus.

Use in Lactation: No adequate studies; caution required.

Pediatric Use: Safety and effectiveness in children below 2 years have not been established.

Geriatric Use: No specific dosage adjustment is necessary for elderly patients.

Renal Impairment: No specific precautions necessary (minimal systemic absorption due to topical use).

Hepatic Impairment: No specific precautions necessary (minimal systemic absorption due to topical use).

Overdose: Overdose is unlikely when used as directed; accidental ingestion: activated charcoal, gastric lavage, artificial ventilation with oxygen, phentolamine to lower blood pressure.

Storage: Store in a cool, dry place, away from heat and direct light. Keep out of reach of children. Do not use more than 4 weeks after opening.
