Description

Eltrombopag Olamine is a small molecule thrombopoietin (TPO) receptor agonist that interacts with the transmembrane domain of the human TPO-receptor, initiating signaling cascades that induce proliferation and differentiation of megakaryocytes from bone marrow progenitor cells, leading to increased platelet production. Used for persistent or chronic ITP, hepatitis C-associated thrombocytopenia, and severe aplastic anemia. Initial dose: 50 mg once daily, adjusted based on platelet counts. Take on empty stomach, at least 2 hours before or 4 hours after calcium-rich foods or polyvalent cation-containing products. Common side effects include nausea, diarrhea, fatigue, headache and elevated liver enzymes.

Pharmacology

Eltrombopag Olamine is a small molecule thrombopoietin (TPO) receptor agonist. Mechanism of action

Eltrombopag interacts with the human TPO receptor transmembrane domain and initiates signaling cascades that induce proliferation and differentiation of megakaryocytes from bone marrow progenitor cells, increasing platelet production.

Dosage

Adult Dose:

  • Chronic ITP: 50 mg once daily, adjust to maintain platelet count ≥50×109/L. Max: 75 mg/day.
  • Chronic hepatitis C: 50 mg once daily, max 100 mg/day.
  • Severe aplastic anemia: 50 mg once daily, adjust to maintain platelet count ≥50×109/L. Max: 150 mg/day.

Child Dose (6 years+): 50 mg once daily. (1-5 years: 25 mg once daily).

Administration

Oral tablet. Take on empty stomach (1 hour before or 2 hours after meals). Take at least 2 hours before or 4 hours after calcium-rich foods or polyvalent cation-containing products. Swallow whole. Do not chew, crush or break.

Missed dose

If you miss a dose of Eltrombopag Olamine, skip it and continue with your normal schedule. Do not double the dose.

Contraindications

Drug interactions

Increased plasma Cmax & AUC of HMG-CoA reductase inhibitors. Caution with methotrexate, topotecan. Increased concentration with fluvoxamine; decreased with rifampicin & lopinavir/ritonavir. Reduced absorption by antacids, dairy products & polyvalent cation-containing products (e.g., Fe, Ca, Mg, Al, Se, Zn).

Food interactions

It is not known whether it is safe to consume alcohol with Eltrombopag Olamine. Please consult your doctor.

Use in pregnancy

Eltrombopag Olamine may be unsafe during pregnancy. Animal studies have shown harmful effects. Doctor will weigh benefits and risks.

Use in lactation

Eltrombopag Olamine is probably unsafe during breastfeeding. Limited human data suggests drug may pass into breastmilk and harm the baby.

Overdose effects

Overdose may cause excessive platelet count increase leading to thrombotic or thromboembolic complications. Consider oral administration of metal cation-containing preparation (calcium, aluminium or magnesium) to chelate eltrombopag and limit absorption.
