Risedronate Sodium + Calcium is a combination used for the treatment and prevention of osteoporosis in postmenopausal women, to increase bone mass in men with osteoporosis, and for Paget's disease of bone.
Description
Risedronate inhibits bone resorption by inhibiting osteoclasts. Calcium is used to prevent or treat negative calcium balance, facilitates nerve and muscle performance, and normal cardiac function. It is a bone mineral component, cofactor in enzymatic reactions, essential for neurotransmission, muscle contraction, and signal transduction pathways.

- Treatment and prevention of osteoporosis in postmenopausal women
- Increase bone mass in men with osteoporosis

Pharmacology
Risedronate inhibits bone resorption by inhibiting osteoclasts. Calcium prevents or treats negative calcium balance, facilitates nerve and muscle performance and normal cardiac function, and plays a key role in bone mineralization and enzymatic reactions.
Mechanism of action
Prevents bone resorption by inhibiting osteoclasts and provides calcium.
Dosage
Adult Dose (Oral):
- Once weekly: 1 risedronate tablet on Day 1, then 1 calcium 500 mg tablet daily with food on the remaining 6 days
Administration
- Risedronate: Take at least 30 minutes before first food of the day with 6-8 oz plain water; do not lie down for 30 minutes after taking
- Calcium: Take with food
Missed dose
If you miss a dose, take it as soon as you remember; skip if it's almost time for next dose; do not double the dose.
Side effects
Side Effects:
- Chest pain
- Difficulty or pain when swallowing
- Pain or burning under the ribs or in the back
- New or worsening heartburn
- Severe joint, bone, or muscle pain
- Jaw pain, numbness, or swelling
- Mild heartburn or stomach upset
- Diarrhea, gas, constipation
- Mild joint or back pain
- Headache
Precautions & warnings
- Correct hypocalcemia before therapy
- Ensure adequate calcium and vitamin D intake
- Avoid in severe renal impairment (CrCl <30 mL/min)
- Do not use in pregnancy and lactation

Drug interactions
- Risedronate: Co-administration with calcium, antacids, or medications containing divalent cations may affect absorption
- Aminoglycosides: Additive calcium lowering effects
- Calcium: Co-administration with thiazide diuretics or vitamin D may lead to milk-alkali syndrome and hypercalcemia; decreased absorption with corticosteroids; decreases absorption of tetracyclines, atenolol, iron, quinolones, alendronate, Na fluoride, Zn, and calcium-channel blockers; enhances cardiac effects of digitalis glycosides
Food interactions
Take risedronate at least 30 minutes before food; calcium with food; it is not known whether it is safe to consume alcohol.
Use in pregnancy
Use in Pregnancy: Do not use during pregnancy; if accidentally taken, stop immediately and consult your doctor.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Do not use during breastfeeding; if accidentally taken, stop immediately and consult your doctor.
Pediatric use
Safety not established in children; consult your doctor.
Geriatric use
Caution required in elderly patients; consult your doctor.
Renal / hepatic impairment
Renal Impairment: Avoid use in severe renal impairment (CrCl <30 mL/min); consult your doctor.
Hepatic Impairment: Caution in liver disease; consult your doctor.
Overdose effects
Overdose: Decreases in serum calcium; signs and symptoms of hypocalcemia; treatment: milk or antacids containing magnesium, calcium, or aluminum; in substantial overdose, gastric lavage may be considered.
Storage conditions
Store in a cool and dry place, protected from light; keep out of reach of children.
