Paliperidone is an atypical antipsychotic used for schizophrenia and schizoaffective disorder. It blocks dopamine D2 and serotonin 5HT2A receptors to improve mood, thoughts and behavior.

Indications

Schizophrenia (acute treatment and recurrence prevention)
Schizoaffective disorder (monotherapy or with antidepressants/mood stabilizers)

Dosage

Adult Dose (Oral):

Schizophrenia: 6 mg/day (initial); increase by 3 mg/day (at intervals >5 days); max 12 mg/day
Schizoaffective disorder: 6 mg/day (initial); 3-12 mg/day; max 12 mg/day

Pediatric Dose (>51 kg): 3 mg/day (initial); 3-12 mg/day; max 12 mg/day

Pediatric Dose (<51 kg): 3 mg/day (initial); 3-6 mg/day; max 6 mg/day

Precautions & warnings

Do not stop suddenly (may worsen symptoms)
Caution in CV disease, QT prolongation, NMS, tardive dyskinesia, hyperglycemia, dyslipidemia, obesity, hyperprolactinemia, leukopenia, neutropenia, agranulocytosis, seizures
Caution while driving
Avoid alcohol
Monitor blood glucose regularly
