Paliperidone is an atypical antipsychotic used for schizophrenia and schizoaffective disorder. It blocks dopamine D2 and serotonin 5HT2A receptors to improve mood, thoughts and behavior.

Indications
  • Schizophrenia (acute treatment and recurrence prevention)
  • Schizoaffective disorder (monotherapy or with antidepressants/mood stabilizers)
Dosage

Adult Dose (Oral):

  • Schizophrenia: 6 mg/day (initial); increase by 3 mg/day (at intervals >5 days); max 12 mg/day
  • Schizoaffective disorder: 6 mg/day (initial); 3-12 mg/day; max 12 mg/day

Pediatric Dose (>51 kg): 3 mg/day (initial); 3-12 mg/day; max 12 mg/day

Pediatric Dose (<51 kg): 3 mg/day (initial); 3-6 mg/day; max 6 mg/day

  • Tachycardia
  • Akathisia
  • Somnolence
  • Tremor
  • Dystonia
  • Cogwheel rigidity
  • Anxiety
  • Weight increased
  • Dyspepsia
  • Constipation
  • Nasopharyngitis
  • Nystagmus
Precautions & warnings
  • Do not stop suddenly (may worsen symptoms)
  • Caution in CV disease, QT prolongation, NMS, tardive dyskinesia, hyperglycemia, dyslipidemia, obesity, hyperprolactinemia, leukopenia, neutropenia, agranulocytosis, seizures
  • Caution while driving
  • Avoid alcohol
  • Monitor blood glucose regularly
